About the Treatment

Ear infections range from a mild, self-limiting outer ear irritation to a middle ear infection needing antibiotics, or a chronic pattern that needs a closer look at what’s keeping it coming back. At CENT, Prof. Dr Khalid Cheema starts by identifying exactly which part of the ear is affected and whether the infection is acute, recurrent, or chronic since the right treatment (and the right level of urgency) depends entirely on that distinction.

CENT treats ear infections in both adults and children from across Lahore including DHA, Iqbal Town, Gulberg, and Johar Town with particular experience managing recurrent childhood ear infections that other treatment hasn’t fully resolved.

What is an Ear Infection?

An ear infection is inflammation and infection affecting part of the ear, most commonly grouped into two types:
Otitis media (middle ear infection) infection behind the eardrum, often following a cold, common in children, typically causing ear pain and sometimes fever
Otitis externa (outer ear / “swimmer’s ear”) infection of the ear canal, often related to moisture, irritation, or minor skin injury from cleaning the ear
Some patients experience recurrent infections (multiple episodes over time) or chronic infections (ongoing, not fully clearing), both of which need a closer diagnostic look rather than repeated short courses of the same treatment.

Symptoms

Common signs of an ear infection include:

Ear pain, ranging from mild to severe

A feeling of fullness or blockage in the ear

Reduced hearing or muffled sound on the affected side

Discharge from the ear

Itching, particularly with outer ear infections

Fever, more common with middle ear infections

In children: pulling or tugging at the ear, irritability, or difficulty sleeping

Redness or swelling of the outer ear or ear canal

Causes

Ear infections can result from a range of causes, including:

Viral or bacterial infection, often following a cold or upper respiratory infection

Water exposure (swimming, bathing) irritating the ear canal

Excessive or aggressive ear cleaning, causing minor skin injury

Eustachian tube dysfunction especially common in children, whose eustachian tubes are naturally narrower

Allergies contributing to fluid buildup behind the eardrum

Wearing hearing aids or earphones for long periods without proper hygiene

Structural issues, such as a perforated eardrum, allowing repeated infection

Diagnosis & Tests

An accurate diagnosis identifies exactly which part of the ear is affected and whether there’s an underlying reason infections are recurring. At CENT, this typically includes:

  • Physical ear examination (otoscopy) to directly view the ear canal and eardrum
  • Tympanometry to check middle ear function and detect fluid buildup
  • Hearing assessment (audiometry), where hearing has been affected
  • Swab/culture of any discharge, where relevant, to guide antibiotic choice
  • Review of infection frequency and pattern key for recurrent cases

Treatment Options

Medical Management

Most ear infections are effectively treated without any procedure:

Antibiotic or antifungal ear drops for outer ear infections

Oral antibiotics for confirmed bacterial middle ear infections

Pain relief and anti-inflammatory medication for symptom control

Ear cleaning (microsuction) in clinic, where debris or discharge is affecting recovery

Myringotomy / Grommets (for Recurrent Cases)

For children (or occasionally adults) with recurrent middle ear infections or persistent fluid buildup affecting hearing, a small procedure to insert grommets (ear tubes) can help drain fluid and reduce the frequency of future infections.

Risks & Complications

Medical management carries minimal risk beyond standard medication considerations, discussed during consultation. Myringotomy/grommet insertion is a well-established, low-risk procedure, with a small chance of temporary discharge, early extrusion, or (rarely) a small perforation that needs monitoring all discussed in detail if this option is recommended.

FAQ

Most acute ear infections improve within a few days of starting appropriate treatment and fully resolve within about a week. Symptoms lasting longer than this, or recurring frequently, are worth having properly evaluated.

Children’s eustachian tubes are naturally narrower and more horizontal, making fluid drainage harder and infections more common. Frequent recurrence is one of the main reasons grommets are considered in children.

Flying or swimming with an active ear infection can worsen discomfort or slow healing, so it’s best to check with your doctor before doing either while symptoms are present.

This is actually a common cause of outer ear infections and irritation, since cotton buds can push wax deeper or injure the ear canal skin. Professional ear cleaning (microsuction) is a safer option when cleaning is genuinely needed.

Not always some resolve with pain relief and monitoring alone, particularly certain viral or mild cases. Your treatment plan will be based on the specific type and severity of the infection.
